ABSTRACT

Introducción: La parálisis cordal bilateral en aducción es la segunda causa de estridor congénito y genera una grave obstrucción de la vía aérea, debutando con estridor. La traqueotomía ha sido durante mucho tiempo el gold estándar para el tratamiento de esta afección, no exenta de complicaciones. Existen procedimientos que intentan evitar la traqueotomía, como el split cricoideo anterior posterior endoscópico (SCAPE). Objetivo: Presentar experiencia con SCAPE en pacientes pediátricos como tratamiento alternativo de parálisis cordal bilateral en aducción. Material y Método: Análisis retrospectivo de los resultados quirúrgicos obtenidos en pacientes con parálisis cordal bilateral en aducción tratados con SCAPE entre enero de 2016 y diciembre de 2019 en el Hospital Guillermo Grant Benavente de Concepción, Chile. Resultados: Siete pacientes se sometieron a SCAPE. Todos los pacientes presentaban insuficiencia respiratoria severa, cinco requirieron asistencia ventilatoria mecánica. Seis pacientes tenían el diagnóstico de parálisis cordal bilateral (PCB) congénita y uno PCB secundaria a tumor de tronco cerebral. Cuatro pacientes presentaron comorbilidad de la vía aérea: dos pacientes presentaron estenosis subglótica grado I y dos pacientes presentaron laringomalacia que requirió manejo quirúrgico. Los días promedio de intubación fueron once días. Ningún paciente requirió soporte ventilatorio postoperatorio, sólo un paciente recibió oxigenoterapia nocturna debido a hipoventilación secundaria a lesión de tronco. Ningún paciente ha presentado descompensación respiratoria grave. Un 40% ha recuperado movilidad cordal bilateral. Conclusión: Split cricoideo anteroposterior endoscópico es una alternativa eficaz para tratar el PCB en pacientes pediátricos. Nuestro estudio evidencia que es una alternativa a la traqueotomía, con excelentes resultados y menor morbimortalidad.


Introduction: Bilateral vocal fold paralysis in adduction is the second cause of congenital stridor and generates a serious obstruction of the airway. Tracheostomy has long been the gold standard for the treatment of this condition, but it has inherent complications. There are procedures that try to avoid tracheotomy, such as the endoscopic anterior posterior cricoid split (EAPCS). Aim: Present our experience with EAPCS in pediatric patients as a treatment for bilateral vocal fold paralysis in adduction. Material and Method: Retrospective analysis of the surgical results obtained in patients with bilateral vocal cord paralysis in adduction treated with EAPCS between January 2016 and December 2019 at Guillermo Grant Benavente Hospital in Concepción, Chile. Results: Seven patients underwent EAPCS. All patients had severe respiratory failure, five required mechanical ventilation assistance. Six patients were diagnosed with congenital bilateral cord palsy (BCP) and one BCP secondary to a brainstem tumor. Four patients had airway comorbidity: two patients had grade I subglottic stenosis and two patients had laryngomalacia that required surgical management. The average days of intubation were eleven days. No patient required post op invasive/non-invasive ventilation, only one patient received nocturnal oxygen therapy due to hypoventilation secondary to trunk injury. None of the patients has presented severe respiratory decompensation. Forty percent have recovered bilateral chordal mobility. Conclusion: SCAPE is a cutting-edge and effective alternative to treat PCB in pediatric patients. Our study shows that it is an alternative to tracheotomy, with excellent results and lower morbidity and mortality.

ABSTRACT

Abstract A male patient was scheduled for urgent amputation of his right forearm. His right forearm was stuck inside the insertion slot of a meat grinder, resulting in severe pain to his injured arm. His upper body could not move to sit in a semi-upright position. An endotracheal tube was successfully placed after rapid sequence intubation using a video laryngoscope from behind the patient on the first attempt. This case report is the first documentation of successful anesthetic induction with subsequent endotracheal intubation using a video laryngoscope from behind an injured patient whose upper body was upright with limited positioning.

ABSTRACT

Objective: To summarize clinical features and our experience of the diagnosis and treatment of laryngocele. Methods: Clinical data of 11 laryngocele patients in department of Otorhinolaryngology Head and Neck Surgery of the Second Affiliated Hospital of Shanxi Medical University from January 2012 to December 2021 were retrospectively reviewed, including 9 men and 2 women, aged from 12 to 75 years, with median age of 56 years. Electronic laryngoscope was performed in 10 of all patients, laryngeal CT in 10 and cervical color ultrasound in 5 before operation.All the operations were performed under general anesthesia, and the external cervical approach was used for external and combined laryngocele. The internal laryngocele was resected by low temperature plasma through transoral endoscopy. Patients were followed up regularly after operation to evaluate the effect. Clinical feature, types of lesions, imaging findings, surgical approaches and follow-up results were analyzed through descriptive statistical method. Results: Eleven laryngocele patients were divided into mixed type (n=6), internal type (n=4) and external type (n=1).Nine patients presented with hoarseness or dysphonia, 7 with cervical mass and 1 with airway obstruction. Surgical resections were done through external cervical approach (n=7)or transoral endoscopic approach (n=4). All the operations were successful and no complication occurred. All cases were followed up from 17 to 110 months. No recurrence was encountered. Conclusions: Laryngocele is a rare lesion with atypical clinical presentation. Preoperative imaging including CT scan and electronic laryngoscope is essential to evaluate the location, and extent of the lesion, and to make the surgical plan.Complete surgical excision is required. Surgical resection is the only effective method for the treatment of laryngocele.

Abstract Background Thyromental height (TMH) was first reported as a great single test for prediction of difficult laryngoscopies, although further studies have shown variable estimates of its accuracy. We thus performed this meta-analysis to summarize the predictive values of TMH mainly for prediction of difficult laryngoscopies. Methods A search in PubMed, EMBASE, LILACS, and Scielo was conducted in June 2020. We included prospective cohorts fully reported with patients ≥ 16 years old, providing data on predictive values of TMH for prediction of either difficult laryngoscopies or difficult intubations. Diagnostic properties and association between TMH and Cormack and Lehanes's classification by direct laryngoscopy were evaluated. A random-effects meta-analysis using hierarchical models was performed. Results Eight studies evaluating 2844 patients were included. All included studies had high risk of bias and low concern regarding applicability. There was significant heterogeneity among the studies. The pooled diagnostic odds ratio (DOR) and positive (LR+) and negative (LR-) likelihood ratios were as follows: DOR, 57.94 (95% CI: 18.19-184.55); LR+, 11.32 (95% CI: 4.28-29.92); and LR-, 0.23 (95% CI: 0.15-0.35). Summary sensitivity and specificity for studies with common threshold were 82.6 (95% CI: 74-88.8%) and 93.5 (95% CI: 79-98.2%), respectively. The estimated AUC was 81.1%. Conclusion TMH arises as a good predictor of difficult laryngoscopies in adult patients from diverse populations presenting better predictive values than most previously reported bedside tests. However, the high risk of bias throughout the studies may have skewed the results of the individual research as well as the summary points of the present meta-analysis.

ABSTRACT

El estridor corresponde a un signo altamente frecuente, sin embargo, es heterogéneo e inespecífico. Existen múltiples causas conocidas y manejadas por el otorrinolaringólogo. Los quistes subglóticos constituyen una entidad infrecuente de estridor en pediatría, siendo la población más frecuentemente afectada, niños con antecedentes de prematurez e intubación por períodos prolongados. Su manifestación clínica es variada, desde cuadros asintomáticos a pacientes con riesgo inminente de pérdida de la vía aérea. El diagnóstico suele ser tras largos períodos desde el antecedente de intubación. Su resolución suele ser quirúrgica, teniendo como principal complicación asociada la estenosis subglótica y las recurrencias. Se presenta el caso de una preescolar con un episodio de estridor y distrés respiratorio rápidamente progresivos, cuyo diagnóstico intraoperatorio resultó en quistes subglóticos submucosos bilaterales, que requirieron resolución quirúrgica.


Stridor corresponds to a highly frequent sign; however, it is heterogeneous and nonspecific. There are multiple causes that are widely known and managed by the otorhinolaryngolo-gist. Subglottic cysts are an infrequent entity of stridor in pediatric patients, where the most frequently affected population are childrens with history of prematurity and intubation for long periods. It's clinical manifestations are wide, from asymptomatic cases to patients with imminent risk of airway loss. Their manifestation its often after long periods after the moment of intubation. The management often involves surgery, and the main associated complication is subglottic stenosis and recurrences. We present the case of a preschool girl with an episode of rapidly progressive stridor and respiratory distress; whose intraoperative diagnosis was bilateral subglottic submucosal cysts that required surgical resolution.

Abstract Background There is currently some discussion over the actual usefulness of performing preoperative upper airway assessment to predict difficult airways. In this field, modified Mallampati test (MMT) is a widespread tool used for prediction of difficult airways showing only a feeble predictive performance as a diagnostic test. We therefore aimed at evaluating if MMT test would perform better when used as a screening test rather than diagnostic. Methods An accuracy prospective study was conducted with 570 patients undergoing general anesthesia for surgical procedures. We collected preoperatively data on sex, age, weight, height, body mass index (BMI), ASA physical status, and MMT. The main outcome was difficult laryngoscopy defined as Cormack and Lahane classes 3 or 4. Bivariate analyses were performed to build three different predictive models with their ROC curves. Results Difficult laryngoscopy was reported in 36 patients (6.32%). Sex, ASA physical status, and MMT were associated with difficult laryngoscopy, while body mass index (BMI) was not. The MMT cut-off with the highest odds ratio was the class II, which also presented significantly higher sensitivity (94.44%). The balanced accuracy was 67.11% (95% CI: 62.78-71.44%) for the cut-off of class II and 71.68% (95% CI: 63.83-79.54) for the class III. Conclusion MMT seems to be more clinically useful when the class II is employed as the threshold for possible difficult laryngoscopies. At this cut-off, MMT shows the considerable highest sensitivity plus the highest odds ratio, prioritizing thus the anticipation of difficult laryngoscopies.

Introducción: la vía aérea difícil es aquella situación clínica en la cual un anestesiólogo entrenado convencionalmente experimenta dificultad en la aplicación de ventilación con mascarilla facial, en la intubación endotraqueal o ambas, es de vital importancia su identificación, puesto que su manejo adecuado marcará la diferencia en el desenlace de los pacientes que requieren intubación, en especial los recibidos en urgencias. Objetivo: revisar de manera sistemática el estado actual del conocimiento y evidencia clínica relacionada al manejo de la vía aérea difícil en in- tubación de emergencia. Material y métodos: se realizó una revisión sistemática en PubMed, Cochrane, EBSCO y OVID; se emplearon los términos manejo de vía aérea difícil e intubación de emergencia. Resul- tados: la búsqueda arrojó 356 resultados, se excluyeron los estudios de revisión sistemática, metaanálisis, artículos basados en opiniones, infor- mes de casos, cartas al editor; 128 artículos fueron analizados; además, se buscó analizar artículos de distinta área de la investigación médica; se seleccionaron 21 artículos para ser analizados en esta revisión sistemática. Conclusiones: inesperadamente los artículos revisados concluyen, en su gran mayoría, que independientemente del protocolo de intubación, el aspecto que juega un rol determinante en el manejo de la vía aérea difícil es la experiencia y preparación previa del médico anestesiólogo (AU)


Introduction: difficult airway is that clinical situation in which a conventionally trained anesthesiologist experiences difficulty in ventilation with a face mask, in endotracheal intubation or both. In this review article we will focus on emergency intubation. Objective: to systematically review the current state of knowledge and clinical evidence related to the management of difficult airways in emergency intubation. Material and methods: a systematic review was carried out in PubMed, Cochrane data base, EBSCO and OVID; the terms: difficult airway management and emergency intubation; only clinical trials and scientific research reports were analyzed. Results: the search yielded 356 results, of which systematic review studies, meta-analysis, opinion-based articles, case reports, letters to the editor were excluded; which gave us 128 articles, after they were analyzed; it was also sought to analyze articles from different areas of medical research; 21 articles were selected to be analyzed in this systematic review. Conclusions: unexpectedly, the majority of the reviewed articles conclude that regardless of the intubation protocol or the tools used, the aspect that plays a decisive role in the management of the difficult airway is the experience and previous preparation of the anesthesiologist (AU))

Abstract Background Both predictions and predictors of difficult laryngoscopy are controversial. Machine learning is an excellent alternative method for predicting difficult laryngoscopy. This study aimed to develop and validate practical predictive models for difficult laryngoscopy through machine learning. Methods Variables for the prediction of difficult laryngoscopy included age, Mallampati grade, body mass index, sternomental distance, and neck circumference. Difficult laryngoscopy was defined as grade 3 and 4 by the Cormack-Lehane classification. Pre-anesthesia and anesthesia data of 616 patients who had undergone anesthesia at a single center were included. The dataset was divided into a base training set (n = 492) and a base test set (n = 124), with equal distribution of difficult laryngoscopy. Training data sets were trained with six algorithms (multilayer perceptron, logistic regression, supportive vector machine, random forest, extreme gradient boosting, and light gradient boosting machine), and cross-validated. The model with the highest area under the receiver operating characteristic curve (AUROC) was chosen as the final model, which was validated with the test set. Results The results of cross-validation were best using the light gradient boosting machine algorithm with Mallampati score x age and sternomental distance as predictive model parameters. The predicted AUROC for the difficult laryngoscopy class was 0.71 (95% confidence interval, 0.59-0.83; p= 0.014), and the recall (sensitivity) was 0.85. Conclusion Predicting difficult laryngoscopy is possible with three parameters. Severe damage resulting from failure to predict difficult laryngoscopy with high recall is small with the reported model. The model's performance can be further enhanced by additional data training.

Introducción: Muchos enfermos de COVID-19 requieren ser ventilados. La laringoscopia directa (LD) es el método tradicional empleado en el manejo de la vía aérea; sin embargo, la videolaringoscopia (VL) es una alternativa en estos pacientes. Objetivo: Comparar la laringoscopia directa y la videolaringoscopia en el manejo de la vía aérea en pacientes con COVID-19. Métodos: Se realizó un estudio observacional retrospectivo. El universo y la muestra estuvieron conformado por los pacientes intubados por médicos de la brigada Henry Reeve en hospitales de Cancún y Ciudad de México. Se conformaron dos grupos; el Grupo laringoscopia directa con 91 pacientes y el Grupo videolaringoscopia con 103. Las variables estudiadas fueron: edad, sexo, número de predictores de una vía respiratoria anatómicamente difícil (VRAD), visualización de la apertura glótica, intentos de intubación y las complicaciones de la intubación. El análisis estadístico de los datos se realizó con el paquete estadístico SPSS 23.0. Resultados: Los grupos fueron comparables en cuanto a edad, sexo y predictores de vía respiratoria anatómicamente difícil. La visualización glótica completa o parcial en el grupo videolaringoscopia fue de 97 por ciento, mientras que en el grupo laringoscopia directa fue de 86 por ciento. La intubación endotraqueal al primer intento superó el 70 por ciento en el grupo VL y el 50 por ciento en el grupo LD. Las principales complicaciones encontradas fueron la desaturación y la hipotensión arterial con una mayor frecuencia en el grupo LD (40,7 por ciento y 49,5 por ciento). Conclusiones: La videolaringoscopia mejoró la visualización glótica y la intubación endotraqueal al primer intento, con menos complicaciones en los pacientes estudiados(AU)


Introduction: Many COVID-19 patients require ventilation. Direct laryngoscopy is the traditional method used for airway management; however, videolaryngoscopy is an alternative in these patients. Objective: To compare direct laryngoscopy and videolaryngoscopy for airway management in COVID-19 patients. Methods: A retrospective observational study was carried out. The universe and the sample consisted of patients intubated by physicians from Henry Reeve brigade at hospitals in Cancun and Mexico City. Two groups were formed: the direct laryngoscopy group, with 91 patients, and the videolaryngoscopy group, with 103 patients. The variables studied were age, sex, number of predictors of an anatomically difficult airway, visualization of the glottic opening, intubation attempts, and intubation complications. Statistical analysis of the data was performed using the SPSS 23.0 statistical package. Results: The groups were comparable in terms of age, sex and predictors of an anatomically difficult airway. Complete or partial glottic visualization in the videolaryngoscopy group was 97 percent , while in the direct laryngoscopy group it was 86 percent . Endotracheal intubation at the first attempt exceeded 70 percent in the videolaryngoscopy group and 50 percent in the direct laryngoscopy group. The main complications observed were desaturation and arterial hypotension with a higher frequency in the direct laryngoscopy group (40.7 percent and 49.5 percent , respectively). Conclusions: Video laryngoscopy improved glottic visualization and endotracheal intubation at the first attempt, with fewer complications in the patients studied(AU)

Resumen Introducción: La disfonía infantil puede afectar negativamente la autoestima del niño y su calidad de vida relacionada con la voz. Objetivo: Describir los resultados del cuestionario Pediatric Voice Handicap Index (pVHI) en niños con patología vocal benigna. Material y Método: Se diseñó un estudio descriptivo en pacientes con patología vocal benigna entre 3 y 15 años en la Unidad de Voz del Hospital de Niños Dr. Luis Calvo Mackenna entre octubre de 2016 y febrero de 2020. La evaluación se realizó mediante un examen laringoscópico y el cuestionario pVHI para evaluar el impacto en la calidad de vida. Resultados: Se incluyeron 49 pacientes, 35 varones (71,4%) y 14 mujeres (28,6%). La edad media fue de 9,27 años. En el examen laringoscópico, el hallazgo más frecuente fueron los nódulos vocales. La puntuación media total en el pVHI fue de 38,77: 11,67 en la subescala o categoría funcional, 18,59 en la física y 8,42 en la emocional. En la evaluación de pVHI por género no existieron diferencias significativas. Conclusión: La patología vocal benigna en la edad pediátrica ocasiona un impacto negativo en la calidad de vida relacionada con la voz. El cuestionario pVHI es un valioso instrumento para evaluar dicha repercusión.


Abstract Introduction: Childhood dysphonia can negatively affect a child's self-esteem and voice-related quality of life. Aim: To describe the results of the Pediatric Voice Handicap Index (pVHI) questionnaire in children with benign vocal fold pathology. Material and Method: A descriptive study was designed in patients with benign vocal fold pathology between 3 and 15 years of age in the Voice Unit of the Dr. Luis Calvo Mackenna Children's Hospital between October 2016 and February 2020. The evaluation was carried out through a laryngoscopic examination and pVHI questionnaire to assess the impact on quality of life. Results: Forty-nine patients were included, 35 boys (71.4%) and 14 girls (28.6%). The mean age was 9.27 years. On laryngoscopic examination, the most frequent finding was vocal fold nodules. The total mean score on the pVHI was 38.77: 11.67 on the functional subscale, 18.59 on the physical subscale, and 8.42 on the emotional subscale. In the evaluation of pVHI by gender, no significant differences were found. Conclusion: Benign vocal fold pathology in pediatric age causes a negative impact on voice-related quality of life. The pVHI questionnaire is a valuable instrument to assess this repercussion.

Resumen La patología quirúrgica de la vía aérea pediátrica suele ser desafiante. Una visualización adecuada de las estructuras faríngeas y laríngeas es absolutamente necesaria para su correcto diagnóstico y tratamiento. Distintos instrumentos, como laringoscopios de intubación, laringoscopios de suspensión y broncoscopios flexibles o rígidos, permiten acceder a la vía aérea. Muchas veces se requiere el uso de una combinación de ellos para abordar con éxito estos problemas. En esta revisión, discutimos el uso de videolaringoscopios en el manejo de condiciones como estenosis subglótica, lesiones de vía aérea y cuerpos extraños. Aunque los anestesiólogos los utilizan frecuentemente para intubaciones difíciles debido a su cámara incorporada que facilita la visión de las estructuras laríngeas, existen escasos informes sobre su uso por cirujanos de vía aérea. Las ventajas sobre la laringoscopía convencional incluyen una mejor visualización, la capacidad de supervisar el procedimiento a través de una pantalla, una mejor ergonomía, que es portátil y que permite una rápida inserción de diferentes instrumentos. Consideramos que es particularmente útil en la dilatación de estenosis subglóticas. Presentamos un método fácil, barato y reproducible para realizarla.


Abstract Surgical pediatric airway diseases are often challenging, and an adequate visualization of pharyngeal and laryngeal structures is absolutely necessary for their correct diagnosis and treatment. Different instruments such as intubation laryngoscopes, suspension laryngoscopes and flexible and rigid bronchoscopes allow for access to the airway, and using a combination of them, is usually required to successfully address these problems. In this review, we discuss the use of videolaryngoscopes in the management of conditions such as subglottic stenosis, airway lesions and foreign bodies. Although commonly used by anesthesiologists for difficult intubations because of their built-in cameras that facilitate the view of laryngeal structures, there are scarce reports on its use by airway surgeons. Advantages over standard laryngoscopy include improved visualization and the ability to supervise the procedure through a screen. We also consider that it allows for improved ergonomics, portability and fast insertion of different instruments. We have found it to be particularly useful in subglottic stenosis dilation and an easy, cheap and reproducible method is also presented.

Los quistes subglóticos adquiridos son una causa rara de estridor en la infancia, cuyo reporte ha ido en aumento en las últimas décadas. Su aparición se relaciona con la prematurez y la intubación en el periodo neonatal. Histológicamente, se observa una obstrucción de las glándulas mucosas de la subglotis debido una metaplasia escamosa del epitelio respiratorio. Esta es una condición que usualmente requiere de un alto índice de sospecha para llegar al diagnóstico, ya que pueden confundirse con otras patologías como laringitis aguda (croup), laringomalacia o asma. La nasofibroscopía permite sospechar su presencia, pero el gold standard diagnóstico lo constituye la laringo-tra-queo-broncoscopía directa en pabellón. Existen diversas técnicas para su manejo, siendo las más frecuentemente utilizadas la marsupialización con instrumental frío y láser. La recurrencia es frecuente, por lo que algunos autores han utilizado mitomicina C y la terapia antirreflujo para intentar disminuirla. Sin embargo, hasta la fecha falta evidencia de calidad que permita llegar a un consenso respecto al manejo ideal de esta patología. En este trabajo, presentamos tres casos clínico de pacientes con antecedentes de prematurez que fueron diagnosticados con quistes subglóticos adquiridos y manejados en un hospital pediátrico de alta complejidad.


Acquired subglottic cysts are an infrequent cause of stridor in infants, which has been increasingly reported in the last decades. Its appearance is related to prematurity and intubation in the neonatal period. Histologically, findings are characterized by an obstruction of the mucosal glands, due to squamous metaplasia of the respiratory epithelium. This condition usually requires a high index of suspicion to be diagnosed, as it can be misdiagnosed as croup, laryngomalacia or asthma. Flexible nasendoscopy allows an initial exploration of the larynx, but direct laryngo-tracheo-bronchoscopy in the operating room is the diagnostic gold standard. There are several techniques for its management, but the most frequently used are cold-steel marsupialization and laser. Recurrence is common, and some authors have used mitomycin C and antireflux therapy to try to decrease it. However, up to date, there is a lack of high-quality evidence, regarding the ideal management of this pathology, which prevents reaching a consensus. In this article, we present three clinical cases of premature patients who were diagnosed with subglottic cysts, treated in a tertiary pediatric hospital.

En pacientes con infección por SARS-CoV-2 la intubación endotraqueal es un procedimiento con riesgo elevado de contagio. La videolaringoscopia complementa la protección del profesional, pero los videolaringoscopios comerciales son caros y no siempre están disponibles en las terapias intensivas pediátricas argentinas. El objetivo fue describir la práctica de intubación en un modelo de cabeza de simulación de lactante con un videolaringoscopio artesanal de bajo costo.Quince pediatras sin experiencia previa con el dispositivo participaron de una práctica de intubación en una cabeza de simulación con un videolaringoscopio artesanal. El tiempo promedio del primer intento fue de 116,4 segundos (intervalo de confianza del 95 % [IC95 %]: 84,8-148,0) y, el del siguiente fue de 44,2 segundos (IC95 %: 27,7­60,6). El tiempo disminuyó de forma significativa en el segundo intento (p : 0,0001). El dispositivo permitió la intubación exitosa en todos los intentos acortando la duración del procedimiento en la segunda práctica


In patients with SARS-CoV-2 infection, endotracheal intubation is a procedure with a high risk for transmission. A videolaryngoscopy is a supplementary level of health care provider protection, but commercial videolaryngoscopes are expensive and not always available in pediatric intensive care units in Argentina. Our objective was to describe intubation practice using an infant head mannequin with a low-cost, handcrafted videolaryngoscope.Fifteen pediatricians with no prior experience using the device participated in an intubation practice in a head mannequin with a handcrafted videolaryngoscope. The average time for the first attempt was 116.4 seconds (95 % confidence interval [CI]: 84.8-148.0) and, for the second one, 44.2 seconds (95 % CI: 27.7-60.6). Time decreased significantly for the second attempt (p: 0.0001).A successful intubation was achieved with the device in all attempts, and the procedure duration decreased with the second practice

ABSTRACT

The laryngoscope is a primary tool that every medical professional who performs in airway care should know how to use. Although it's an irreplaceable tool, in the last decade have been considered transformations and modifications in them that have allowed the medical professional, especially the anesthesiologist to perform successfully for critical management especially in the context of unanticipated difficult airway, because most of the time the only available resource at hand is the laryngoscope and a malleable stylet that guides the orotracheal. The new technological age of visual instruments such as video cameras and photographs many of these with small sizes, has been used in the manufacture of video laryngoscopes that today are used, which have managed to approach the aerea in a minimally traumatic way and also safeguard lives. Our hydrid video laryngoscope is a handcrafted tool made of high-quality elements, strength, durability and low cost.


El laringoscopio es una herramienta primordial que todo profesional médico que se desempeña en la atención de la vía aérea debería saber utilizar. Pese a ser un dispositivo insustituible, en la última década se han considerado transformaciones y modificaciones en los mismos que le han permitido al profesional de la medicina, fundamentalmente al anestesiólogo, desempeñarse exitosamente durante el manejo crítico especialmente en el contexto de vía aérea difícil no anticipada, en el que la mayoría de las veces el único recurso disponible a la mano es el laringoscopio y un estilete maleable que sirve de guía para dirigir la sonda orotraqueal. La nueva era tecnológica de los instrumentos visuales como cámaras de video y fotografía, (muchos de estos con tamaños reducidos) han sido empleados en la fabricación de videolaringoscopios que hoy en día se utilizan y que han logrado abordar la vía aérea de manera mínimamente traumática, así mismo, salvaguardar vidas. Nuestro videolaringoscopio híbrido es una herramienta artesanal fabricada con elementos de alta calidad, resistencia, durabilidad y bajo costo.

ABSTRACT

INTRODUÇÃO: A consulta pré-anestésica é de extrema importância para o médico anestesiologista no planejamento do manejo das vias aéreas de pacientes sob o efeito de anestesia geral com intubação orotraqueal (IOT). OBJETIVO: Avaliar a sensibilidade, especificidade, valor preditivo positivo/negativo (VPP/VPN) de testes de predição de IOT difícil (Escore de Wilson - EW, e Teste de Mallampati modificado - TMM), em pacientes submetidos à anestesia geral, em hospital filantrópico do interior de Minas Gerais. MATERIAIS E MÉTODOS: Estudo descritivo transversal, por meio de fichas pré-anestésicas e transoperatórias, de pacientes submetidos à anestesia geral com IOT, entre os meses de janeiro (2019) e março (2020). RESULTADOS: Dos 440 pacientes, 56,1% necessitaram de IOT: média de idade de 49,9 anos (desvio padrão 18,6). A maioria foi classificada: TMM classe I e II; pontuação 0 a 2 no EW; distância esternomentoniana >12,5 cm, sugerindo IOT fácil. Apenas o TMM apresentou correlação com IOT difícil (p=0,045). Sensibilidade e especificidade dos testes respectivamente: TMM (54,6%;75,9%); EW (36,4% e 79,7%); baixo VPP (TMM: 9,5%; EW: 7,7%) e alto VPN (TMM: 97,3%; EW: 96,4%). Curva ROC: área sob a curva foi de TMM = 0,68; EW = 0,60. CONCLUSÃO: Apesar do TMM apresentar correlação significativa com a IOT difícil, não foi possível definir o melhor teste preditor. Ressaltamos que a sensibilidade e o VPP, de ambas as avaliações, ficaram abaixo daquilo que seria considerado adequado para um teste de rastreio e predição.


Introduction: A pre-anesthetic appointment is extremely important for the anesthesiologist when planning the management of the airways of patients under the effect of general anesthesia with orotracheal intubation (OTI). Objective: To evaluate the sensitivity, specificity, positive/ negative predictive value (PPV/NPV) of difficult OTI prediction tests (Wilson risk-sum ­ WRS, and Modified Mallampati Test - MMT) in patients undergoing general anesthesia in a philanthropic hospital in the countryside of the state of Minas Gerais. Materials and methods: Descriptive cross-sectional study using pre-anesthetic and transoperative records of patients submitted to general anesthesia with OTI between the months of January (2019) and March (2020). Results: Of the 440 patients, 56.1% required OTI: average age of 49.9 years (standard deviation 18.6). Most classified: MMT class I and II; score 0 to 2 on the WRS; sternomental distance greater than 12.5 cm, suggesting easy OTI. Only MMT showed statistical significance with difficult OTI (p=0.045). Sensitivity and specificity of the tests respectively: MMT (54.6%; 75.9%) WRS (36.4% and 79.7%) low PPV (MMT: 9.5%; WRS: 7.7%) and high NPV (MMT: 97.3%; WRS: 96.4%). ROC Curve: area under the curve was MMT = 0,68; WRS = 0,60. Conclusion: Although the MMT has a significant correlation with the difficult OTI, it was not possible to define the best predictor test. We emphasize that the sensitivity and PPV of both evaluations were below what would be considered adequate for a screening and prediction test.

Abstract Background and objectives When planning the management of a predicted difficult airway, it is important to determine which strategy will be followed. Video laryngoscopy is a major option in scenarios with factors suggesting difficult airway access. It is also indicated in rescue situations, when there is tracheal intubation failure with direct laryngoscopy. The objective of the present report was to show the efficacy of using the video laryngoscope as the first device for a patient with a large tumor that occupied almost the entire anterior portion of the oral cavity. Case report An 85 year-old male patient, 162 cm, 70 kg, ASA Physical Status II, Mallampati IV classification, was scheduled for resection of an angiosarcoma located in the right maxillary sinus that invaded much of the hard palate and the upper portion of the oropharynx. He was conscious and oriented, with normal blood pressure, heart and respiratory rates and, despite the large tumor in the oral cavity, he showed no signs of respiratory failure or airway obstruction. After intravenous cannulation and monitoring, sedation was performed with 1 mg of intravenous midazolam, and a nasal cannula was placed to provide oxygen, with a flow of 2 L min−1. Then, the target-controlled infusion of remifentanil with an effect site concentration of 2 ng mL−1 was initiated, according to Minto's pharmacokinetic model. Ventilation was maintained spontaneously during airway handling. A trans-cricothyroid block was performed, with 8 mL of 1% lidocaine solution injected into the tracheal lumen. Slight bleeding did not prevent the use of an optical method for performing tracheal intubation. The entire oral cavity was sprayed with 1% lidocaine. The McGraph video laryngoscope with the difficult intubation blade was used, and an armored tube with a guide wire inside was used for tracheal intubation, performed on the first attempt with appropriate glottis visualization. Conclusion The video laryngoscope occupies a prominent position in cases in which access to the airway is difficult. In the present case it was useful. It can be used as first choice or as a rescue technique. The video laryngoscope is an appropriate alternative and should be available for facing the ever-challenging difficult airway patient.


Resumo Justificativa e objetivos No planejamento da abordagem a uma via aérea difícil prevista, é importante determinar qual será a estratégia a ser seguida. A videolaringoscopia é uma ótima opção em situações em que existam fatores indicadores de dificuldade de acesso à via aérea. Também é indicada em situações de resgate, quando houve insucesso na tentativa de intubação com a laringoscopia direta. O objetivo deste relato é mostrar a eficácia da utilização do videolaringoscópio como primeiro dispositivo diante de paciente com grande tumor que ocupava quase a totalidade da porção anterior da cavidade oral. Relato do caso Paciente com 85 anos, sexo masculino, 162 cm, 70 kg, estado físico ASA II, classificação de Mallampati IV, foi escalado para a ressecção de um angiossarcoma localizado no seio maxilar direito que invadia grande parte do palato duro e da porção superior da orofaringe. Apresentava-se lúcido, consciente e orientado, com valores de pressão arterial, frequência cardíaca e respiratória normais e, apesar do grande tumor na cavidade oral, não apresentava qualquer sinal de insuficiência respiratória ou de obstrução das vias aéreas. Após venóclise, foi feita monitorização e sedação com 1 mg de midazolam, por via venosa, e colocado cateter nasal para administração de oxigênio, com fluxo de 2 L.min-1. Em seguida, foi iniciada a infusão alvo-controlada de remifentanil com concentração efeito de 2 ng.mL-1 segundo o modelo farmacocinético de Minto. A ventilação foi mantida em espontânea durante a manipulação da via aérea. Foi realizado bloqueio transcricotireóideo, sendo injetados 8 mL de solução de lidocaína a 1% na luz traqueal. Um pequeno sangramento não impediu que um método óptico fosse utilizado para realizar a intubação traqueal. Toda a cavidade oral recebeu o spray de lidocaína tópica a 1%. Foi utilizado o videolaringoscópio McGraph com a lâmina de intubação difícil, e um tubo aramado com fio guia no seu interior, foi utilizado para a intubação traqueal, que foi realizada na primeira tentativa, com boa visualização da glote. Conclusão O videolaringoscópio ocupa uma posição de destaque nos casos em que o acesso à via aérea é difícil. No presente caso, a sua utilização foi útil. Ele pode ser utilizado como primeira opção ou como técnica de resgate. Nas condições sempre preocupantes diante de um paciente com via aérea difícil, o videolaringoscópio deve estar disponível, constituindo-se uma boa opção.

Abstract Introduction Mitomycin C is a natural antibiotic that has been used to inhibit the proliferation of fibroblasts in scar tissue. Objective To evaluate the effectiveness and safety of topical Mitomycin C as an adjuvant in the endoscopic treatment of laryngotracheal stenoses. Data synthesis A systematic review of experimental or observational studies that have evaluated the treatment of laryngotracheal stenoses with the use of topical Mitomycin C was performed. Databases researched: LILACS, PubMed, Embase, Cochrane and Web of Science. Outcomes: resolution (symptom-free time ≥ one year), number of procedures required, and complications resulting from the procedure. A total of 15 studies (involving 387 patients) were selected. Mitomycin C was administered to every patient in 11 studies, and in 4 other studies, the patients were separated into 2 groups, 1 receiving mitomycin C, and the other not. The resolution of the stenosis evaluated in 12 studies in which the patients received mitomycin C was of 69% (95% confidence interval [95%CI]: 61-76%; I2 = 17.3%). A total of 52% of the patients (95%CI: 39-64%, 11 studies; I2 = 64.7%) were submitted to a single endoscopic procedure, and 48% (95%CI: 36-61%, 11 studies; I2 = 64.7%) were submitted to more than 1 procedure. Complications (mediastinal and subcutaneous emphysema, dysphonia, laceration or vocal fold paralysis and acute light obstruction) were reported in 9% of the patients (95%CI: 3-18%, 9 studies; I2 = 79.8%). Conclusions The evidence suggests that mitomycin C is an effective and safe option in the endoscopic treatment of laryngotracheal stenosis.

RESUMEN Introducción: En la valoración de dificultad para realizar la laringoscopia convencional no se realza la integración necesaria de aspectos clínicos esenciales relacionados con el control respiratorio. Objetivo: Validar un modelo de predicción de una laringoscopia anatómicamente difícil en el paciente que requiere de intubación orotraqueal. Método: Se realizó un estudio analítico en una población de 17 966 pacientes con necesidad de laringoscopia directa para una intubación orotraqueal con fines quirúrgicos en el Hospital General Docente "Dr. Agostinho Neto" de Guantánamo entre el 2015 y el 2018. Se determinó por muestreo aleatorizado una muestra de 17 068 pacientes. Se estudiaron las siguientes variables: estigma periférico para laringoscopia difícil, laringoscopia difícil pos-inducción anestésico, evaluación laringoscópica según Cormack-Lehane, valor diagnóstico del modelo de evaluación predictiva para laringoscopia. Resultados: Los altos grados en la clasificación de aspectos clínicos predictivos y la coexistencia con la alteración morfológica de la epiglotis fueron los marcadores más asociados con la probabilidad de laringoscopia anatómicamente difícil. Con la integración de cuatros aspectos clínicos esenciales se identificó el grado de dificultad probable para visualizar las cuerdas vocales. Conclusiones: Se diseñó un modelo que posibilitó la predicción de una laringoscopia anatómicamente difícil, cuya validación certificó su viabilidad para aplicarlo en la práctica médica.


ABSTRACT Introduction: In assessing the difficulty of performing conventional laryngoscopy, the necessary integration of essential clinical aspects related to respiratory control is not enhanced. Objective: To validate a prediction model of an anatomically difficult laryngoscopy in the patient that requires orotracheal intubation. Method: An analytical study was carried out in a population of 17,966 patients in need of direct laryngoscopy for an orotracheal intubation for surgical purposes at the General Teaching Hospital "Dr. Agostinho Neto" from Guantanamo between 2015 and 2018. A sample of 17,068 patients was determined by randomized sampling. The following variables were studied: peripheral stigma for difficult laryngoscopy, difficult laryngoscopy after anesthetic induction, laryngoscopic evaluation according to Cormack-Lehane, diagnostic value of the predictive evaluation model for laryngoscopy. Results: The high degrees in the classification of predictive clinical aspects and the coexistence with the morphological alteration of the epiglottis were the markers most associated with the probability of anatomically difficult laryngoscopy. With the integration of four essential clinical aspects, the degree of probable difficulty in visualizing the vocal cords was identified. Conclusions: A model was designed that allowed the prediction of an anatomically difficult laryngoscopy, whose validation certified its feasibility to apply it in medical practice.

Objetivo: Validar um modelo de previsão de uma laringoscopia anatomicamente difícil no paciente que necessita de intubação orotraqueal. Método: Foi realizado um estudo analítico em uma população de 17.966 pacientes com necessidade de laringoscopia direta para intubação orotraqueal para fins cirúrgicos no Hospital Geral de Ensino "Dr. Agostinho Neto" de Guantánamo entre 2015 e 2018. Uma amostra de 17.068 pacientes foi determinada por amostragem aleatória. Foram estudadas as seguintes variáveis: estigma periférico para laringoscopia difícil, laringoscopia difícil após indução anestésica, avaliação laringoscópica segundo Cormack-Lehane, valor diagnóstico do modelo de avaliação preditiva para laringoscopia. Resultados: Os altos graus na classificação dos aspectos clínicos preditivos e a coexistência com a alteração morfológica da epiglote foram os marcadores mais associados à probabilidade de laringoscopia anatomicamente difícil. Com a integração de quatro aspectos clínicos essenciais, foi identificado o grau de provável dificuldade na visualização das cordas vocais. Conclusões: Foi elaborado um modelo que permitia prever uma laringoscopia anatomicamente difícil, cuja validação atestava sua viabilidade de aplicá-la na prática médica.

RESUMO Objetivo Este estudo teve por objetivo comparar os achados videoendoscópicos da deglutição orofaríngea em distintas consistências de alimento na Esclerose Lateral Amiotrófica (ELA). Método Estudo clínico retrospectivo com amostra de conveniência. Foram incluídos 20 indivíduos com diagnóstico de ELA e disfagia orofaríngea confirmada por avaliação clínica e objetiva de deglutição, independentemente do tipo, bulbar ou esquelética, e tempo de diagnóstico neurológico, 13 do sexo masculino e sete do sexo feminino, faixa etária variando de 34 a 78 anos, média de 57 anos. Foram analisados os achados da videoendoscopia de deglutição (VED) nas consistências líquida (N=18), líquida espessada (N=19) e pastosa (N=20) no volume de cinco ml. Os achados sobre escape oral posterior, resíduos faríngeos, penetração laríngea e/ou aspiração foram comparados nas três consistências de alimento e a análise estatística utilizou o teste ANOVA de Friedman. Resultados Não foi encontrada alteração na sensibilidade laríngea nessa população. Não houve diferença estatística significativa entre as consistências de alimento na presença de escape oral posterior, penetração e/ou aspiração. Houve diferença estatística significativa somente com resíduos faríngeos na consistência líquida espessada e pastosa na ELA. Conclusão Dentre os achados videoendoscópicos da deglutição na ELA, somente o resíduo faríngeo teve maior frequência na dependência da consistência de alimento.


ABSTRACT Purpose This study aimed to compare the fiberoptic endoscopic findings of oropharyngeal swallowing of distinct food consistencies in Amyotrophic Lateral Sclerosis (ALS). Methods This was a retrospective clinical study of a convenience sample of 20 individuals (13 males and seven females aged 34 to 78 years old) with a diagnosis of ALS and oropharyngeal dysphagia confirmed by clinical and objective evaluation of swallowing, regardless of the bulbar or skeletal type and of the time of neurological diagnosis. The fiberoptic endoscopic evaluation of swallowing (FEES) of the liquid (N = 18), thickened liquid (N = 19) and pureed samples (N = 20) in a volume of 5 ml were analyzed. Data related to posterior oral spillage, pharyngeal residues, laryngeal penetration and/or aspiration after swallowing the three food consistencies were analyzed statistically by the Friedman ANOVA test. Results No impairment of laryngeal sensitivity was found in this population. There was no statistically significant difference in posterior oral spillage, penetration and/or aspiration between food consistencies. There was a statistically significant difference only related to pharyngeal residues of the thickened liquid and pureed consistency. Conclusion Among the fiberoptic endoscopic findings of swallowing in ALS, only pharyngeal residues had a higher frequency depending on the consistency of food.
